Greetings!

I love the game, it is an awesome serious sim. But as far as I have seen it is not just me, but some other players are confused with the way departments work. It is hard to understand what does a clinic part for a department mean, and why can there be no common areas, waiting rooms.
For me, something based on real life would be more logical. The game correctly tries to suggest, that the outpatient and inpatient health service are two different activities, and in-game this corresponds to clinic and hospitalization part.
But as far as I know (at least here in Hungary), there is no such thing as a hospital department itself having a separate outpatient service as well, besides in-patent hospitalization.

The way it should mimic real life, is that an outpatient clinic is a separate independent institution or one is attached to a hospital as a department, which only deals with outpatients. But a clinic itself has besides GPs all the specialists for basic diagnostics, so it has - based on the in-game departments - a cardiology specialist doctors office, an internal medicine specialist doctors office, a neurologist doctors office etc. but it does not have in-patient facilities, wards, operating rooms. And all the other hospital departments are for in-patient care, and some special diagnostics procedures. They do not need a specialist basic office, because the patient are sent there from the outpatient clinic specialist if they require department hospitalization, or some special equipment in diagnostics.

So in practice this would mean changing the department structure (no separate clinic and hospitalization within a department) and facilities to a list like this:

General areas (commonly used in all departments): Reception, Waiting Room, Toilets, Common Room, Corridor, Cleaning closet - it would be a very welcomed change if this would be handled as common areas, and the patients the janitors, staff would use it without regards to departmental affiliations

Outpatient clinic: the basic department, and without other (in-patient) departments it makes the medical institution an independent clinic and not a hospital
Possible rooms:  General Practitioners Office, Internal Medicine Office, Cardiology Office, Neurology Office, Orthopedic Office

Radiology: the same as in game, a special diagnostics department, which receives patents from all other departments
       Possible rooms:  X-ray room, CT room, MRI room, CAG room

Central lab: this should be a common department as radiology, and all the samples are sent from other departments
Possible rooms: General Laboratory, Molecular Laboratory etc. some more specialized rooms, but not a partitioning based on medical fields, because that makes no sense

Emergency department/Traumatology: by itself, it does only receive mobile patents with critical state and immobile patents with the ambulance as currently, but it does not provide normal out-patent diagnostics
Possible rooms: Observation room, Trauma center, Nurses station, On-call room

General Surgery department: the same without the clinic aspect
Possible rooms: Nurses station, On-call room, Regular Ward, HDU Ward, Operating Room, Diagnostic unit, Sonography unit

Internal medicine department: the same without the clinic aspect
Possible rooms: Nurses station, On-call room, Regular Ward, HDU Ward, Operating Room, Diagnostic unit, Cardiography unit

Orthopedic department: the same without the clinic aspect
Possible rooms: Nurses station, On-call room, Regular Ward, HDU Ward, Operating Room, Diagnostic unit, Sonography unit

Cardiology department: the same without the clinic aspect
Possible rooms: Nurses station, On-call room, Regular Ward, HDU Ward, Operating Room, Diagnostic unit, Sonography unit, Cardiography unit

Neurology department: the same without the clinic aspect
Possible rooms: Nurses station, On-call room, Regular Ward, HDU Ward, Operating Room, Diagnostic unit, Neuroexam unit

ICU department: the same
Possible rooms: Nurses station, On-call room, ICU
